AdvanceMed, the Medicare Program Safeguard Contractor for Medicare Part A and Part B in Ohio, recently issued a bulletin indicating that Medicare providers are not correctly reporting their practice location information on their Medicare enrollment applications.

A provider must list all locations where the provider renders services to Medicare beneficiaries, including each hospital where the provider performs services. For example, if a physician renders services to Medicare beneficiaries at the physician’s office as well as two hospitals where the physician has privileges, all three locations must be reported as practice locations on the physician’s Medicare enrollment form. A provider may be denied entry or revoked from the Medicare program for not meeting Medicare enrollment requirements.

Providers who believe that their current Medicare enrollment information omits one of the provider’s practice locations should submit a Change of Information form to update the provider’s enrollment.
