A surveyor’s assistant has suffered significant injuries after a piece of plastic grid mesh over a shaft penetration gave way and he fell nearly eight metres down the  mine shaft.

The Court found that the shaft wasn’t barricaded or marked with warning signs and the pieces of mesh were not secured together or marked to prohibit workers standing on them.

The three Western Australian companies that made up the joint venture were fined a total of $75,000 for OHS breaches.
