ICO has fined Prudential £50,000 for a serious breach of the Data Protection Act, when it mixed up the accounts of two customers for three years. The original error was caused when the records of both customers, who share the same first name, surname and date of birth, were mistakenly merged in March 2007. After that Prudential failed to correct the mistake, despite being told about it several times. (Source: Prudential Fined £50,000 for Customer Account Confusion)
