Great Western Hospitals NHS Trust have been fi ned £75k after admitting breaching section 3 (1) HSWA 1974 following the death of Mayra Cabrera, a nurse, at the hospital. After giving birth, Mrs Cabrera was inadvertently given an epidural drug in her arm instead of a saline solution, which led to her death. Investigations carried out by the HSE and the Police following the incident established that drugs were not stored under a clear system, and that earlier incidents which highlighted the defi ciencies had been ignored. This increased the risk of human error when selecting and administrating drugs, as similar drugs were stored in the same cupboards without an adequate system being in place. The trust admitted putting patients at risk as a result of the inadequate storage and administration of drugs, and was also ordered to pay costs of £25k in addition to the fi ne.
