The Ohio Department of Insurance and the Ohio Association of Health Plans recently unveiled a new standardized health insurance application form – the Ohio Uniform Small Group Application (App 1210). The form is intended to simplify the health insurance application process for employers seeking quotes from multiple carriers, making it easier for insurance agents, and especially employers and employees who will now only have to fill out the form once.

The Ohio Uniform Small Group Application form may be used by all Ohio licensed carriers or health insuring corporations and does not need to be filed individually by each company with the department.
