A report which details the investigation carried out by the HSE and the Environment Agency following the events in December 2005 has been produced which includes previously unpublished information which can only be made available now as the criminal investigation is complete. The failings that were noted included the fact that the systems used to fill the industrial petrol tanks were both deficient and not fully implemented, and that a culture had developed whereby keeping operations going was considered more important than the safety processes, which were not sufficiently prioritised.
