AL2012-041-E, U.S.A Healthcare Psychiatric Services, LLC d/b/a The Sanctuary at them Woodlands, Cullman, AL: This application was filed for an Emergency Certificate of Need to relocate 20 psychiatric beds from Hartselle Medical Center, which closed January 31, 2012, to The Sanctuary at the Woodlands. This is being presented to the Certificate of Need Review Board for ratification.
- AL2012-033, Dialysis Clinic, Inc.-Moulton, Moulton, AL: Proposes to add two additional hemodialysis stations to the existing 10-station dialysis facility, for a total of 12 stations, one of which will be used as an isolation station.
- AL2012-036, Chateau Vestavia Hills, LLC, Birmingham, AL: Proposes to convert eight assisted living apartments into eight specialty care assisted living apartments. The eight specialty care assisted living beds are being relocated from Faith Assisted Living and were approved under
Change of Ownership 2012-016.

Administrative Law Judge filed an order denying the project. Opposition: Baptist Health System, Inc. d/b/a Princeton Baptist Medical Center filed opposition/intervention and request for a Contested Case Hearing.

Rural, Marshall Medical Center South Hospital requests to establish a Physician Clinic and Infusion Center for Medical Oncology and will be owned by a physicians group to be located on the campus of Marshall Professional Center.

RV2012-034, Sawrie Oncology, Inc. requests proposed assignment of professional services contracts from South Alabama Radiation Oncology, P.C., Monroeville Radiation-Oncology, P.C. and Gulf Coast Cancer Center-Gulf Shores, P.C. (collectively, the “Physician Groups”) to Sawrie Oncology, Inc. Following the assignment, Sawrie will continue to provide the same outpatient radiation oncology services that the Hixson Physician Groups currently provides, at the same locations, and the Hixson Physician Groups will cease to provide these health services altogether.
