AL2016-006, University of South Alabama Medical Center, Fairhope, AL: Proposes to relocate the existing USA Medical Center Fairhope Outpatient Department to new leased space approximately a quarter of a mile from its existing leased space at 188 Hospital Drive, Ste. 400, Fairhope, AL. B. AL2016-007, Dale County Health Care Authority d/b/a Dale Medical Center, Ozark, AL: Proposes to initiate a Swing Bed Program by certifying ten (10) of its acute care beds into the Center for Medicare and Medicaid Services Swing Bed Program. Opposition: None.
C. AL2016-009, Rehabilitation Hospital of Phenix City, LLC d/b/a Regional Rehabilitation Hospital, Phenix City, AL: Proposes to expand the existing forty-eight (48) licensed bed capacity by adding ten (10) new inpatient rehabilitation beds pursuant to a Plan Adjustment approved for Russell County, Region V, by Gov. Bentley. Opposition: None.
D. Al2016-010, Bio-Medical Applications of Alabama, Inc. d/b/a Fresenius Medical Care Valley Creek, Selma, AL: Proposes to add six (6) hemodialysis stations to the existing ESRD facility consisting of sixteen (16) stations, for a total of twenty-two (22) stations, pursuant to Chapter 410-2-3-.05(2) of the Alabama State Health Plan. RV2016-013, Homestead Hospice requests to consolidate two parent providers, currently located in Choctaw and Dallas Counties, into one surviving parent provider (Dallas County) and one satellite provider (Choctaw County).
