Flowserve (GB) Ltd of West Sussex was fined £150,000 after pleading guilty to breaching s 2(1) of the Health and Safety at Work Act 1974 with regard to the May 2008 death of a 21-year-old employee, Philip Locke. Mr Locke died of chest injuries when he was hit by a high pressure valve which had become detached from a machine and hit him at high speed. The HSE commented that the death could have easily been prevented had an adequate risk assessment been made and the system of work used been modified. Flowserve (GB) Ltd was also ordered to pay full costs of £66,838 by Lewes Crown Court. (click here)
